MSP Staff Physician Appointments
Overview
An MSP Staff Physician is a staff (non-academic) employee who also holds a concurrent academic without salary (WOS) appointment. These positions address a clinical need that currently cannot be met by faculty members.
About the MSP Staff Physician position
The Academic Affairs Office of the School of Medicine (SOM) is the UCSF governing authority for the Staff Physician Policy. The office approves the staff title, salary, and academic appointment for MSP Staff Physicians.
Important points and requirements
- Primary Staff Appointment: MSP Staff Physicians are staff employees and must be hired under an MSP Staff Physician staff title code
- Concomitant Academic Appointment: MSP Staff Physicians must have a concurrent without salary (WOS) academic appointment. The without salary (WOS) academic appointment is established to ensure proper malpractice coverage and to enable the performance of teaching functions related to the physician’s clinical work. The WOS academic appointment cannot continue without an active staff appointment.
- Concurrent Appointments. MSP Staff Physicians cannot hold concurrent paid academic appointments, including internships, clinical fellowships, residencies or faculty appointments. They may hold concurrent Career Tracks staff appointments after open recruitment and subject to review by the Dean’s office.
- Licensing: Must have a California medical license.
- Board Certification: Board certification is required only for specific title codes (those with the Diplomate designation). To qualify for those title codes, board certification must be in the discipline in which the physician will be practicing.
- No Research: MSP Staff Physicians cannot engage in research. If the position involves research duties, the department should work with their Academic Personnel Team to identify an appropriate paid academic appointment type. MSP Physicians may serve as site director to facilitate patient enrollment in clinical trials on studies for which a faculty member serves as the PI.
- Compensation: Pay for MSP Staff Physicians typically consists of two components: base pay and non-base pay. Base pay paid at the minimum for the title code, serving as covered compensation for retirement. Non-base pay is additional compensation above the base pay to comprise the total pay rate for the physician. Non-base pay is not covered compensation for retirement. Other compensation components may include shift differential pay, flat rate shifts and clinical incentives. For more details, see the MSP Staff Physician Compensation section below.
- Employee Relations: Employee relations issues related to MSP Staff Physicians fall under the responsibility of Labor and Employee Relations in consultation with UCSF Staff HR Shared Services.

Determine staff employee classifications

The appointment type — also referred to as Employee Class in UCPath — is typically determined based on the expected effort and time commitment for the physician assignment.

Contract appointments are typically established for one-year terms and may be renewed annually. Employees in contract appointments of one year or more and FTE of 50% or more are eligible for full UC benefits. MSP Physician appointments are exempted from the five-year maximum included in Personnel Policies for Staff Members (PPSM) 3, and may be extended for any number of years.
- Limited Appointment: For physicians appointed at 43% effort or less. Can be for a fixed or variable schedule. Physicians in these appointments typically are not eligible for benefits.
- Per Diem Appointment: For physicians who will work as needed on a variable schedule and who will be paid by agreement. This is commonly used for physicians who are asked to provide as-needed coverage and will be paid a shift rate for work performed. Physicians in these appointments are not eligible for benefits.

Compensation
This section describes the common components of MSP Staff Physician compensation and the process for establishing an approved pay plan for MSP Staff Physicians. Pay plans are reviewed and approved by the School of Medicine, Academic Affairs Office. Please refer to the School of Medicine Staff Physician procedure for additional information about the guidelines for MSP Staff Physician compensation.
Base pay
The MSP Staff Physician base pay is the minimum pay requirement associated with the job code selected for the position. The base pay (SPR/REG) rate and the non-base pay (SPN) rate together equal the overall rate the department is paying a particular physician for that service. The base pay (SPR/REG) wages count towards retirement and the non-base pay (SPN) wages do not.
Minimum salary requirements (REG/SPR) effective as of July 1, 2026:
Associate Physician Series
- Associate Physician Diplomate (000770): $93.19 hourly, $16,212 monthly, $194,544 annually
- Associate Physician (000771): $83.14 hourly, $14,467 monthly, $173,604 annually
Senior Physician Series
- Senior Physician Diplomate (000768): $93.19 hourly, $16,212 monthly, $194,544 annually
- Senior Physician (000769): $83.14 hourly, $14,467 monthly, $173,604 annually
Non-base pay
Non-base pay (SPN) is a component of the total salary for an MSP Physician. Non-base pay is any amount above the base pay (REG/SPR) established for the applicable job code to make up the total salary negotiated with the MSP Physician.
To calculate the annual non-base pay, subtract the annual base pay rate from annual overall rate to determine non-base pay. To convert an annual amount to the monthly rate, divide by 12. To convert an annual amount to an hourly rate, divide by 2,088.
When submitting your requisition in Oracle, use the annual rate for physicians working a fixed schedule and use the hourly rate for physicians who will be working a variable schedule.
Other pay components
Departments may include other pay components for their Staff Physicians in their approved compensation plans. This can include by-agreement payments as well as clinical incentive payments.
By-agreement payments
By-agreement payments may be established where standard pay rates do not feasibly address the unique characteristics, demands, and duration of the shift. There must also be a demonstrated difficulty in obtaining clinical coverage. Payment of a flat rate for similar services must be consistent with market practice. Common examples of by-agreement payments are:
- Flat rate for full night on call
- Weekend or Night shift differentials
- Full shift rates for physicians who are called to work as needed for operational needs (e.g., $1,000 for weekday shift and $1,500 for weekend shift)
Clinical incentive pay
Clinical Incentive Plans (CIPs) are incentive plans designed to acknowledge and reward MSP Staff Physicians with 20% or greater effort who excel in clinical services. CIPs may not exceed 20% of the annual REG earnings, a limit set by the UC Regents. CIP payments cannot be based upon non-base pay earnings. The 20% limit on the REG is based on the actual annual earnings based on FTE, not the annual pay rate. A clinical incentive plan rewards individuals who have achieved specific and quantifiable performance objectives set by the department.
The clinical incentive plan document should plan specify who is eligible to participate, the criteria by which the incentives are determined, and the level(s) of clinical incentives possible. The plan document and associated payments are reviewed and approved by the School of Medicine’s Academic Affairs office.
